July 10, 2001 – Thirtymile Fire
Four firefighters were fatally injured and four more firefighters and two civilians were injured on the Thirtymile Fire in the Chewuch River Canyon of the Okanogan-Wenatchee National Forest. Northwest Regulars #6 crewmembers Karen FitzPatrick, Jessica Johnson, Dennis Weaver, and squadleader Tom Cravens were killed on a previously-thought safe rock scree slope when the fire ran up both sides of the canyon, entrapping the crew and civilians.
Many changes were enacted after this tragedy, including enforcing the 2:1 work/rest cycle, ensuring all fireline personnel are qualified for the ICS position they are performing, requiring written evaluations on all out-of-area resources on Type 3, 4, and 5 fires, and documenting adherence to LCES, the 10 Standard Fire Orders, and mitigation measures for the 18 Watchout Situations. In addition, the Cantwell-Hastings bill, which was signed into law in 2002, requires the Department of Agriculture's Inspector General's Office to investigate any U.S. Forest Service fire fatality.
