completed NOW is the time to get it done.

You May Not try-out unless you have this form completed by a Doctor.

Monday, October 4, 2010 – Sandia HS

Monday, October 25, 2010 – Cibola HS

TIMES: 5:00pm to 7:00pm

(100 students -- maximum capacity)

PRICE: $25.00

(cash or check made to APS Athletics)

Open to any APS student – student must be accompanied by a parent or guardian or have all parental signatures completed on the 

physical exam packet prior to exam time
