What about the incident up north in 2008 involving the helicopter?  Of course, I do understand your list may be limited to those who perished because of fire behavior, however that was not clear. I am not sure that the fire community would discount those who lost their lives in 2008 as not incident related deaths and continue to refer to this incident as Line of Duty Death (LODD).
